ADIME + 3-Day Meal Plan

Service Description

Everything offered with an ADIME plus a personalized meal plan for three days! If possible, please email, or have your medical provider(s) email your available personal health information to me@ifythedietitian.com PRIOR to your appointment. Payment is due at the time of booking. This service may be covered by your health insurance plan! In order for the service to be covered by your health insurance, a referral from your medical provider which shows the need for Medical Nutrition Therapy (MNT) is usually required. Check with your insurance plan to determine if/what nutrition services are covered by your plan. If you are covered, I will send you a reimbursement form to give to your insurance provider. Please have your medical provider utilize the referral form on the services page (or utilize their own referral document) and email the completed form to me@ifythedietitian.com. If you do not have health insurance, don't worry! You can still receive this service! As always, I will do my best to provide high-quality nutritional care based on the information that is available to me.
