Service Description

ADIME stands for Assessment, Diagnosis, Intervention, Monitoring and Evaluation. Looking to control your blood sugars, alleviate depression or lose weight? Most ailments and concerns can be positively impacted by adopting healthier nutrition habits. This service will provide you with personalized, evidence-based recommendations to help you achieve realistic, measurable goals. Once this service is booked, you will receive an email from me containing a questionnaire that will help me understand you, your health status and current goals/desires. The questionnaire is a detailed nutrition assessment that will cover several topics including health history, anthropometric data, family health history, current medication regimen, food preferences, diet restrictions and your lifestyle. Next, based on the assessment information, I will create one or more nutritional diagnoses which will help pin-point current or potential nutritional problems and the source(s) of these problems. Finally, I will create measurable, personalized recommendations (interventions) that will help alleviate or cure nutritional issues and bring you closer to your goals. My responses to your assessment will be sent back to you via email so you can keep them handy for as long as you need. The ADIME Monitoring and Evaluation service allows us to monitor your progress towards your goals and re-evaluate the recommendations to see what needs to be changed. If possible, please email, or have your medical provider(s) email your available personal health information to me@ifythedietitian.com. Payment is due at the time of booking. This service may be covered by your health insurance plan! In order for the service to be covered by your health insurance, a referral from your medical provider which shows the need for Medical Nutrition Therapy (MNT) is usually required. Check with your insurance plan to determine if/what nutrition services are covered by your plan. If you are covered, I will send you a reimbursement form to give to your insurance provider. Please have your medical provider utilize the referral form on the services page (or utilize their own referral document) and email the completed form to me@ifythedietitian.com. If you do not have health insurance, don't worry! You can still receive this service! As always, I will do my best to provide high-quality nutritional care based on the information that is available to me.
